General Summary:
Under the general direction of the Clinical Director, the Vocational Case Manager assists in the delivery of quality services and engaging activities that meet the individual’s needs, supports the HOPE Center Manager in day to day operations in compliance with regulatory requirements, and shares responsibility for electronic record-keeping.
Overview:
The Vocational Case Manager carries a caseload of clients whose primary focus is in obtaining a job. This position builds and maintains relationships with the clients and works to establish long term consistent relationships with local employers. This position attends job fairs, sets up meetings with perspective employers, has client sessions and serves as a liaison between the placed employee and employer.
Essential Requirements:
1. Life experience essential for insight into the population. Preferably a Bachelors Degree in Social Work, Psychology or a complimentary field
2. Ability to work in and adapt the culture of a multi-cultural, community based organization
3. Familiar with the communities within JOURNEYS service area
4. Familiar with prospective employers in the area
5. Ability to maintain appropriate boundaries with staff and clients in all circumstances
6. Good inter-personal skills, ability to relate well with persons from a variety socioeconomic and cultural groups.
7. Subscription to and integration of the agency’s core values of Respect, Teamwork, Mission-Driven and Commitment
8. Technological competency

Essential Functions:
DIRECT CLIENT SERVICES | 65%
• Conduct individual sessions with those who are in need of continued assistance in the area of referral and linkage to local employment opportunities.
• Refer clients to other HOPE Center services including, but not limited to, the PDI Housing Readiness Program and the Mental Health Program, if necessary.
• Assist in the outreach process by informing the homeless population of the services provided at the Hope Center.
• Participate in the case review process.
• Stay current on all documentation of services.
• Complete all required forms for each session for client goal planning, client statistic gathering and input purposes
• Work closely with the Case Managers, Mental Heath Therapist, Volunteer Supervisors and volunteer crew to coordinate center services
NETWORKING | 25%
• Interface with other non-profits in the area to assure seamless client service referral
• Attend workshops and trainings to assure best practices
• Meet with area employers to determine if they may be a good referral source for our clients
• Serve as liaison between the employee placement and the employer
• Attend local job fairs
ADMINISTRATIVE FUNCTIONS | 10%
• Research prospective employers in the area

This position will require exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
