The HOPE Center is a free homeless resource center serving individuals and families experiencing homelessness or at risk of homelessness in the Northwest suburbs of Chicago. Located in Palatine, Illinois, the HOPE Center provides comprehensive support services designed to help people find stability, secure housing, and connect with essential community resources.
Services include a food pantry, shower and laundry facilities, mail services, transportation assistance, and personalized case management. Case management services include housing counseling, housing assistance programs, employment and job counseling, mental health support, healthcare referrals, legal service referrals, and connections to local social service resources.
The HOPE Center also serves as the access point for emergency shelter registration in the surrounding area, helping individuals and families connect with available shelter options. In addition, a family nurse practitioner provides free healthcare services every Wednesday and can assist clients with accessing medications and prescription resources.
Whether you need emergency shelter, homelessness prevention services, housing support, or referrals to community resources, the HOPE Center is committed to helping people move from crisis to stability and permanent housing.
New clients should plan to arrive before 3:00 PM, Monday through Friday, to allow sufficient time to complete the intake process and receive available services.

PATHWAYS HOUSING READINESS
The shortage of affordable housing in Northwest suburban Cook County has created significant challenges for individuals and families experiencing homelessness or housing instability. To help address this growing community need, JOURNEYS | The Road Home created Pathways Housing Readiness, a housing assistance program that helps people experiencing homelessness prepare for and secure permanent housing.
Pathways Housing Readiness provides a clear, step-by-step pathway from homelessness to stable housing. The program helps individuals and families become housing-ready through personalized case management, housing counseling, life-skills support, and connections to local housing resources. Participants receive referrals to affordable housing opportunities, rental assistance programs, and other services that promote long-term housing stability.
Once housed, clients continue receiving support from JOURNEYS' clinical team through ongoing counseling, goal setting, and individualized care plans. This supportive housing approach helps participants maintain permanent housing, improve overall well-being, and work toward greater independence.

When home is out of reach, recovery shouldn't be too. For people experiencing homelessness, a hospital discharge doesn't mean the hard part is over. Without a safe, stable place to rest and recover, even the best medical care can unravel. JOURNEYS Medical Respite changes that.
We provide short-term supportive shelter for individuals recovering from acute medical issues who no longer need hospitalization but who aren't yet ready to fully care for themselves on their own. We're here to give healing a chance.
JOURNEYS Medical Respite offers a structured, supportive environment where clients can focus on getting better. Our team works alongside each person to address not just their immediate medical needs, but the bigger picture of long-term stability.

Who We Serve
JOURNEYS Medical Respite is designed for adults 18 and older who are experiencing homelessness and recovering from an acute medical condition — but who are medically stable and able to care for themselves independently.
Individuals are eligible if they:
- Are 18 or older and currently experiencing homelessness
- Are medically stable and do not require inpatient or detox services
- Can independently perform daily living activities and manage their own medications
- Are psychologically stable with no active risk of harm to themselves or others
- Are willing to engage in case management and follow program guidelines

For Referring Partners
Hospitals, shelters, and service providers play a critical role in connecting people to the care they need. JOURNEYS Medical Respite accepts referrals from healthcare and social service professionals who identify individuals who meet our eligibility criteria.
To make a referral, please have the following ready:
- Completed referral form
- 30-day supply of the client's medications
- Supporting documentation relevant to the individual's care and situation
The individual being referred must meet eligibility criteria and be willing to actively participate in their own care and recovery.
