The HOPE Center is a resource center that provides free services to the homeless and those at risk of becoming homeless.
Services include food pantry, laundry and shower facilities, mailbox, transportation assistance, and case management which covers job counseling, mental health counseling, housing counseling, housing assistance, and healthcare and legal referrals. The HOPE Center is also where people register to use emergency shelters in the area and a family nurse practitioner offers free healthcare on Thursdays and can help accessing prescriptions.
New clients should plan to arrive before 3 PM Monday through Friday to have enough time to complete the intake process.

PATHWAYS HOUSING READINESS
The affordable housing shortage in northwest suburban Cook County has caused hardship and crisis for many individuals and families. JOURNEYS launched Pathways Housing Readiness to address this critical need in the community and help people in a step-by-step process to get into permanent housing.
Pathways Housing Readiness offers hope for someone ready to move out of homelessness, preparing them to become housing-ready while providing referrals and placement in stable housing. Once housed, the clinical team provides clients with the necessary support and counseling to help clients reach their goals.

A Safe Place to Heal
When home is out of reach, recovery shouldn't be too.For people experiencing homelessness, a hospital discharge doesn't mean the hard part is over. Without a safe, stable place to rest and recover, even the best medical care can unravel. JOURNEYS Medical Respite changes that.
We provide short-term supportive shelter for individuals recovering from acute medical issues who no longer need hospitalization but who aren't yet ready to fully care for themselves on their own. We're here to give healing a chance.
JOURNEYS Medical Respite offers a structured, supportive environment where clients can focus on getting better. Our team works alongside each person to address not just their immediate medical needs, but the bigger picture of long-term stability.
While in our program, clients receive:

Who We Serve
JOURNEYS Medical Respite is designed for adults 18 and older who are experiencing homelessness and recovering from an acute medical condition — but who are medically stable and able to care for themselves independently.
Individuals are eligible if they:
- Are 18 or older and currently experiencing homelessness
- Are medically stable and do not require inpatient or detox services
- Can independently perform daily living activities and manage their own medications
- Are psychologically stable with no active risk of harm to themselves or others
- Are willing to engage in case management and follow program guidelines

For Referring Partners
Hospitals, shelters, and service providers play a critical role in connecting people to the care they need. JOURNEYS Medical Respite accepts referrals from healthcare and social service professionals who identify individuals who meet our eligibility criteria.
To make a referral, please have the following ready:
- Completed referral form
- 30-day supply of the client's medications
- Supporting documentation relevant to the individual's care and situation
The individual being referred must meet eligibility criteria and be willing to actively participate in their own care and recovery.

There are many ways to get involved at JOURNEYS to help people experiencing homelessness and housing insecurity in the community. The rise in homelessness over the past few years has increased demand and volunteers are critical to ensuring JOURNEYS can help as many people as possible.
